Overview

acalabrutinib + venetoclax + durvalumab is a combination therapy being investigated by the Mayo Clinic for the treatment of Richter transformation (Richter syndrome) arising from chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL). This regimen combines three distinct therapeutic mechanisms: durvalumab, a monoclonal antibody that acts as an immune checkpoint inhibitor by targeting PD-L1; acalabrutinib, a second-generation Bruton's tyrosine kinase (BTK) inhibitor that blocks B-cell receptor signaling; and venetoclax, a BCL-2 inhibitor that promotes apoptosis in malignant B-cells. The combination aims to overcome the aggressive nature and poor prognosis of Richter syndrome by simultaneously targeting survival pathways and enhancing the immune system's ability to detect and destroy cancer cells.
