Acapatamab + abiraterone is an experimental combination therapy under investigation for metastatic castration-resistant prostate cancer (mCRPC). It comprises acapatamab, a bispecific T-cell engager antibody targeting prostate-specific membrane antigen (PSMA) and CD3, and abiraterone acetate, a small-molecule androgen biosynthesis inhibitor. Abiraterone suppresses androgen production, which is critical for prostate cancer growth, by irreversibly inhibiting the enzyme CYP17. Acapatamab is designed to redirect T cells to PSMA-expressing prostate cancer cells, promoting their killing via immune-mediated mechanisms. This regimen is being tested in patients with mCRPC who have progressed on previous standard therapies.
