Overview

Basel_insulin is a combination therapy for type 2 diabetes mellitus, composed of the alpha-glucosidase inhibitor acarbose and a basal insulin (such as insulin glargine, detemir, or degludec). Acarbose acts locally in the gut to competitively and reversibly inhibit pancreatic alpha-amylase and membrane-bound intestinal alpha-glucosidase, delaying the digestion and absorption of carbohydrates, thereby reducing postprandial blood glucose spikes. Basal insulin provides a steady background level of insulin to control fasting and between-meal blood glucose, but may not adequately control postprandial hyperglycemia; the addition of acarbose primarily targets postprandial glucose excursions. This combination is indicated for patients with type 2 diabetes inadequately controlled with basal insulin alone and has shown additive effects in lowering hemoglobin A1c and postprandial glucose. Common side effects are gastrointestinal and related to acarbose use, including flatulence and abdominal discomfort, mostly mild and transient. Basal insulin, meanwhile, carries the risk of hypoglycemia and weight gain. The combination is not a fixed-dose product but refers to the concurrent use of two established antidiabetic agents[1][3][5][10].
