Drug intelligence / Profile preview
Acenocoumarol (also known as acenocoumarin) is a coumarin-derived oral anticoagulant that acts as a vitamin K antagonist. It is widely used in Europe, India, and other regions for the prevention and treatment of thromboembolic disorders, including deep vein thrombosis, pulmonary embolism, and stroke in patients with atrial fibrillation or mechanical heart valves. Acenocoumarol works by inhibiting the enzyme vitamin K epoxide reductase (VKOR), which prevents the reduction of vitamin K epoxide to its active hydroquinone form. This depletion of active vitamin K impairs the gamma-carboxylation and activation of vitamin K-dependent clotting factors II, VII, IX, and X, as well as anticoagulant proteins C and S. Compared to warfarin, acenocoumarol has a shorter elimination half-life, which can lead to a more rapid onset and offset of action, though it still requires careful monitoring of the International Normalized Ratio (INR) due to its narrow therapeutic index and susceptibility to genetic variations in CYP2C9 and VKORC1.
