acetaminophen + carmustine is an investigational combination chemotherapy regimen evaluated by Memorial Sloan Kettering Cancer Center for the treatment of stage III and stage IV metastatic melanoma. In this protocol, high-dose acetaminophen is administered as a chemosensitizing agent to deplete intracellular glutathione (GSH) levels. Glutathione is a primary cellular antioxidant that protects cells from oxidative stress and detoxifies certain chemotherapy drugs, including alkylating agents. By reducing GSH concentrations, acetaminophen enhances the DNA-alkylating activity and overall cytotoxicity of carmustine (BCNU) against melanoma cells. This strategy is designed to sensitize tumor cells that are otherwise resistant to nitrosourea-based chemotherapy. The regimen typically includes the administration of acetylcysteine as a rescue agent to replenish glutathione in normal tissues and mitigate potential hepatotoxicity from the high-dose acetaminophen.
