Overview

Acetaminophen + diphenhydramine + phenylephrine is an over-the-counter (OTC) combination medication used for the temporary relief of symptoms associated with the common cold, influenza, and upper respiratory allergies. It consists of three small molecule active ingredients: **acetaminophen**, a non-aspirin analgesic and antipyretic; **diphenhydramine**, a first-generation antihistamine with sedative and antimuscarinic properties; and **phenylephrine**, a sympathomimetic decongestant. Acetaminophen works by inhibiting prostaglandin synthesis in the central nervous system and acting on the hypothalamic heat-regulating center to reduce fever and pain. Diphenhydramine competitively blocks H1 receptors to alleviate runny nose and sneezing, while its sedative effects often make the combination suitable for nighttime use. Phenylephrine acts as a potent vasoconstrictor by stimulating alpha-1 adrenergic receptors, thereby reducing swelling in the nasal passages and relieving sinus pressure. The combination is available in various oral formats including caplets, liquids, and powders, and is generally indicated for adults and children 12 years of age and older.
