Acetaminophen + propylene glycol is an investigational drug combination studied by Beth Israel Deaconess Medical Center for the prevention of acetaminophen-induced liver injury. In this context, propylene glycol serves as a competitive inhibitor of the cytochrome P450 2E1 (CYP2E1) enzyme. CYP2E1 is the primary metabolic pathway responsible for converting acetaminophen into its reactive and hepatotoxic metabolite, N-acetyl-p-benzoquinone imine (NAPQI). By inhibiting this enzyme, propylene glycol aims to reduce NAPQI formation and mitigate liver damage. This combination was evaluated in a Phase 4 crossover clinical trial (NCT02339142) where healthy volunteers received high-dose acetaminophen alongside propylene glycol to assess its protective efficacy against subclinical hepatotoxicity. The 'Second Treatment Period' designation refers to a specific phase within this crossover study design.
