acetazolamide + eszopiclone + venlafaxine

acetazolamide + eszopiclone + venlafaxine (RESCUE-Combo) is an investigational combination therapy being evaluated by the University of California, San Diego for the treatment of obstructive sleep apnea (OSA) in patients who are intolerant to continuous positive airway pressure (CPAP). This regimen utilizes an endotype-targeted approach to address the physiological causes of OSA. Acetazolamide, a carbonic anhydrase inhibitor, acts to stabilize ventilatory control by reducing 'loop gain.' Eszopiclone, a non-benzodiazepine sedative-hypnotic (GABA-A receptor agonist), is included to increase the respiratory arousal threshold. Venlafaxine, a serotonin-norepinephrine reuptake inhibitor (SNRI), is optionally added to enhance upper airway muscle activity during sleep. Results from the Phase 2 RESCUE-Combo trial demonstrated that the combination significantly reduced the apnea-hypopnea index (AHI) and was generally well-tolerated, representing a potential pharmacological alternative for CPAP-intolerant patients.
