Overview

Acetazolamide + prednisolone is a combination of two pharmaceutical agents, each with distinct mechanisms of action. Acetazolamide is a carbonic anhydrase inhibitor that reduces the formation of hydrogen and bicarbonate ions from carbon dioxide and water, leading to diuresis, decreased intraocular pressure, and altered acid-base balance. It is primarily used for glaucoma, edema (including heart failure-related), certain types of epilepsy, and altitude sickness[2][4][8]. Prednisolone is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory and immunosuppressive properties; it acts by binding to the glucocorticoid receptor to modulate gene expression involved in inflammation and immune response[5]. The combination may be used in clinical scenarios where both reduction in fluid retention/pressure (acetazolamide) and suppression of inflammation or immune activity (prednisolone) are desired—such as severe visual loss associated with pseudotumor cerebri/papilledema[3]. However, this combination should be used cautiously due to potential drug interactions affecting electrolyte balance and risk for adverse effects such as muscle cramps or confusion[1][6].
