Overview

These drugs inhibit the PDE5 enzyme to increase cyclic guanosine monophosphate (cGMP) levels, leading to smooth muscle relaxation and increased blood flow—primarily used for erectile dysfunction but also studied in other vascular disorders. - **Carnitine** (including L-carnitine or acetyl-L-carnitine), a naturally occurring amino acid derivative involved in mitochondrial fatty acid transport. It acts as an antioxidant and has been investigated for improving sperm motility in male infertility, neuroprotection, metabolic support, and other uses. The combination of PDE5 inhibitors with antioxidants such as carnitines or acetylcysteine is under investigation primarily for male reproductive health issues like erectile dysfunction or idiopathic male infertility. Evidence suggests that combining PDE5 inhibitors with antioxidants may improve outcomes compared to monotherapy[1][2][4]. The mechanisms involve reducing oxidative stress (acetylcysteine/carnitines) alongside enhancing nitric oxide-mediated vasodilation (PDE5 inhibition).
