**Acetylsalicylic acid + proton pump inhibitor** is a fixed-dose combination medication designed to provide cardiovascular protection while reducing the risk of gastric and duodenal ulcers in patients requiring long-term antiplatelet therapy. Acetylsalicylic acid, commonly known as aspirin, is an antiplatelet agent that prevents platelet aggregation and reduces the risk of heart attack and stroke. Proton pump inhibitors (PPIs), such as omeprazole or esomeprazole, decrease gastric acid production, providing gastroprotection against ulcers and bleeding caused by aspirin therapy[3][7][8]. The combination is primarily indicated in patients at increased gastrointestinal risk due to chronic aspirin use for prevention of cardiovascular or cerebrovascular events. Mechanistically, aspirin irreversibly inhibits cyclooxygenase-1 (COX-1) in platelets (reducing thromboxane A2), while PPIs inhibit gastric H+/K+ ATPase, blocking acid secretion. The combination is available in branded forms such as Yosprala (aspirin + omeprazole); generic and other fixed-dose combinations may also exist for acetylsalicylic acid with other PPIs[3][8]. Clinical studies demonstrate that the combination reduces the incidence of endoscopy-proven peptic ulcers and patient-reported dyspeptic symptoms compared to aspirin alone, with tolerability similar to individual agents[3]. It is generally administered orally, once daily, in a delayed-release tablet to protect stomach lining[7][8].
