acitretin + pioglitazone

Acitretin + pioglitazone is a combination of two small molecule drugs used experimentally for the treatment of moderate to severe chronic plaque-type psoriasis. Acitretin is an oral retinoid that modulates keratinocyte differentiation and proliferation by binding to nuclear retinoic acid receptors, leading to normalization of epidermal cell growth. Pioglitazone is a thiazolidinedione class drug that acts as an agonist at peroxisome proliferator-activated receptor gamma (PPAR-γ), primarily used in diabetes mellitus type 2 but also shown to have anti-inflammatory and antipsoriatic effects. The combination has demonstrated greater efficacy than acitretin alone in reducing Psoriasis Area and Severity Index (PASI) scores, with a manageable safety profile over short-term use[1][2][3][5]. This regimen may be particularly useful for patients with coexisting metabolic syndrome or diabetes.
