Acitretin + tripterygium wilfordii glycoside is a combination therapy consisting of two agents: - **Acitretin** is an oral retinoid (vitamin A derivative) primarily used for the treatment of severe psoriasis. It acts by modulating epidermal cell proliferation and differentiation. - **Tripterygium wilfordii glycoside (TWG)** is an extract from the traditional Chinese medicinal plant *Tripterygium wilfordii* Hook F ("Thunder God Vine"), containing multiple bioactive compounds such as triptolide and celastrol. TWG exhibits anti-inflammatory and immunosuppressive effects through inhibition of pro-inflammatory cytokines, suppression of Th17 cell function via STAT3 pathway inhibition, promotion of regulatory T cells (Tregs), and induction of T cell apoptosis. The combination has been studied in clinical trials for moderate to severe plaque psoriasis (MSPP), where it demonstrated superior efficacy and safety compared to monotherapy with either agent alone[1][2][3][4]. The rationale for combining these drugs lies in their complementary mechanisms—acitretin's modulation of keratinocyte activity with TWG's broad immunomodulatory actions.
