ACX-02 (Applied Cognition)

**ACX-02** is an investigational, fixed-dose small-molecule combination developed by Applied Cognition to enhance sleep-associated glymphatic clearance of Alzheimer disease-related proteins. The clinically studied regimen combines intravenous dexmedetomidine, a central alpha-2 adrenergic agonist that suppresses central noradrenergic tone and increases slow-wave sleep, with oral midodrine, an alpha-1 adrenergic agonist prodrug used to maintain systemic arterial pressure. In a completed Phase 1/2 crossover study in healthy older adults, this pharmacologic approach increased plasma measures of brain-to-blood amyloid beta and tau clearance during a controlled sleep opportunity.
