Ad26.Mos.HIV + MVA-Mosaic is a heterologous prime-boost HIV-1 vaccine regimen developed by Janssen (Johnson & Johnson). The regimen utilizes two distinct viral vectors: a recombinant adenovirus serotype 26 (Ad26.Mos.HIV) and a modified vaccinia Ankara (MVA-Mosaic) vector. Both components are engineered to express 'mosaic' HIV-1 antigens—bioinformatically optimized sequences of Gag, Pol, and Env proteins designed to elicit broad immune responses against the high genetic diversity of global HIV-1 strains. While the regimen demonstrated robust immunogenicity in early-phase trials (APPROACH, TRAVERSE), large-scale efficacy trials for HIV prevention, including the Phase 2b IMBOKODO study and the Phase 3 MOSAICO study, were terminated after data indicated the vaccine did not provide sufficient protection against HIV infection. It has also been investigated as a therapeutic vaccine component in people living with HIV.
