Ad26.RSV.preF is an investigational vaccine candidate developed by Janssen (a Johnson & Johnson company) for the prevention of Respiratory Syncytial Virus (RSV) infection and associated lower respiratory tract disease (LRTD). The vaccine utilizes a unique dual-component approach within a single injection, combining an adenovirus serotype 26 (Ad26) viral vector that expresses the RSV fusion (F) protein stabilized in its prefusion conformation with a recombinant RSV prefusion F protein subunit. This design is intended to stimulate both potent cellular (T-cell) and humoral (antibody) immune responses, mimicking a prime-boost strategy. The vaccine demonstrated approximately 80% efficacy against RSV-mediated LRTD in older adults during Phase 2b (CYPRESS) and Phase 3 trials. However, in March 2023, Johnson & Johnson announced the discontinuation of its RSV vaccine program to prioritize other assets in its pipeline.
