Adalimumab + etanercept is a hypothetical combination of two biologic disease-modifying antirheumatic drugs (DMARDs), both of which are tumor necrosis factor-alpha (TNF-α) inhibitors. Adalimumab is a fully human monoclonal antibody that binds to TNF-α, preventing its interaction with cell surface TNF receptors and thereby reducing inflammation. Etanercept is a dimeric fusion protein consisting of the extracellular ligand-binding portion of the human p75 TNF receptor fused to the Fc portion of IgG1; it acts as a decoy receptor for TNF-α, removing it from circulation and inhibiting its pro-inflammatory effects. Both agents are approved individually for several autoimmune diseases, including rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, and plaque psoriasis[3][5][8]. The use of these two agents in combination is not standard clinical practice due to increased risk of serious infections and immunosuppression[7]. There are no approved products or clinical guidelines supporting their combined use.
