The components are: - **Adalimumab**: A monoclonal antibody that inhibits tumor necrosis factor alpha (TNFα), reducing inflammation in conditions like rheumatoid arthritis, Crohn's disease, and others[2][7]. - **Etanercept**: A fusion protein acting as a TNF inhibitor by binding to TNFα and preventing its interaction with cell surface receptors[1][4]. - **Hydroxychloroquine**: An antimalarial agent also used as a DMARD; it modulates immune system activity. - **Infliximab**: A chimeric monoclonal antibody against TNFα, used for autoimmune diseases such as RA and Crohn’s disease[3][4]. - **Leflunomide**: An immunomodulatory agent that inhibits pyrimidine synthesis, leading to reduced lymphocyte proliferation. - **Methotrexate**: A folate analog metabolic inhibitor that suppresses inflammation by inhibiting enzymes involved in nucleotide synthesis; widely considered the anchor drug for RA therapy[6][8]. - **Sulfasalazine**: An anti-inflammatory DMARD thought to modulate local chemical mediators of the inflammatory response. These drugs are sometimes combined in clinical practice for patients with severe or refractory rheumatoid arthritis. However, combining multiple biologics (such as adalimumab plus etanercept plus infliximab) is generally not recommended due to increased risk of serious infections. More common combinations involve methotrexate with one or more conventional DMARDs (hydroxychloroquine, leflunomide, sulfasalazine), or methotrexate plus a single biologic agent[9].
