This combination immunotherapy regimen, developed by Roswell Park Cancer Institute, is designed to overcome resistance to PD-1/PD-L1 checkpoint inhibitors in patients with HLA-A2+ melanoma. The regimen consists of an autologous alpha-type-1 polarized dendritic cell (aDC1) vaccine and a chemokine-modulating (CKM) component. The CKM component is a triple combination of interferon alpha-2b, rintatolimod (a double-stranded RNA TLR3 agonist), and celecoxib (a COX-2 inhibitor). The treatment aims to reprogram the tumor microenvironment by inducing the expression of T-cell-attracting chemokines (such as CXCL9, CXCL10, and CCL5) and reducing immunosuppressive factors like prostaglandin E2, thereby facilitating the infiltration of cytotoxic T lymphocytes into the tumor and restoring sensitivity to checkpoint blockade.
