This investigational combination regimen consists of the PD-L1 monoclonal antibody adebrelimab, a metronomic chemotherapy cocktail known as VEX (comprising oral vinorelbine, cyclophosphamide, and capecitabine), and radiotherapy. It is currently being evaluated in a Phase II clinical trial (NCT06771609) for the treatment of advanced triple-negative breast cancer (TNBC). Adebrelimab acts as an immune checkpoint inhibitor by blocking the interaction between PD-L1 and its receptors, thereby restoring T-cell anti-tumor activity. The VEX regimen utilizes low-dose, frequent administration of chemotherapy to exert anti-angiogenic and immunomodulatory effects rather than direct cytotoxicity. Radiotherapy is integrated into the regimen to potentially induce an abscopal effect and provide synergistic immunostimulatory signals that enhance the efficacy of the PD-L1 inhibitor in the TNBC microenvironment.
