Overview

Adefovir dipivoxil is an orally administered acyclic nucleotide analog reverse transcriptase inhibitor (ntRTI) used for the treatment of chronic hepatitis B. It is a prodrug that is converted in vivo to adefovir and then phosphorylated to its active form, adefovir diphosphate. The active metabolite inhibits hepatitis B virus (HBV) DNA polymerase by competing with deoxyadenosine triphosphate and causing DNA chain termination after incorporation into viral DNA. This results in decreased HBV replication[2][3][4]. Adefovir also has immunomodulatory effects and can improve T cell immune function in patients with chronic hepatitis B[6]. The drug was developed by Gilead and has been marketed under the brand name Hepsera[5]. The "hepatitis B vaccine" refers to recombinant protein vaccines that induce immunity against HBV infection by stimulating the production of protective antibodies against the surface antigen of HBV. This entry represents a combination of an antiviral small molecule (adefovir dipivoxil) and an immunoprophylactic biologic (hepatitis B vaccine), which are sometimes used sequentially or together for management or prevention strategies related to hepatitis B but are not available as a fixed-dose combination product.
