Adelumab + VEX is an investigational combination therapy comprising adelumab, a monoclonal antibody targeting Programmed Death-Ligand 1 (PD-L1), and the VEX metronomic chemotherapy regimen, which consists of vinorelbine, cyclophosphamide, and capecitabine. This combination is being evaluated by the Cancer Hospital, Chinese Academy of Medical Sciences, for the treatment of advanced HER2-negative breast cancer with brain metastases. Adelumab functions as an immune checkpoint inhibitor, enhancing the anti-tumor immune response by blocking the interaction between PD-L1 and its receptor PD-1. The VEX regimen utilizes low-dose, frequent administration of cytotoxic agents to exert anti-angiogenic and immunomodulatory effects while maintaining a favorable safety profile. The combination is often studied alongside radiotherapy to potentially synergize with the abscopal effect and improve intracranial disease control.
