Overview

This is a combination of three pharmacological agents—adenosine, eptifibatide, and nitroglycerin—used primarily in the context of acute coronary syndromes or percutaneous coronary intervention (PCI). - **Adenosine** is an endogenous nucleoside that acts as a vasodilator, particularly in the coronary circulation. It increases arterial endothelial cell nitric oxide via activation of adenosine A2a receptors on vascular smooth muscle cells, leading to potent but short-lived vasodilation and increased coronary blood flow[8]. - **Eptifibatide** is a small-molecule glycoprotein IIb/IIIa inhibitor that reversibly blocks platelet aggregation by preventing fibrinogen and other adhesive ligands from binding to the GP IIb/IIIa receptor on platelets. This reduces thrombus formation during acute ischemic events[1][6][7]. - **Nitroglycerin** (often spelled "nitroglyzerin" in error) is an organic nitrate that acts as a vasodilator by releasing nitric oxide, relaxing vascular smooth muscle, dilating both arteries and veins (with greater effect on veins), reducing myocardial oxygen demand, and increasing blood supply to ischemic myocardium[8]. This combination may be used off-label or investigationally for synergistic effects during PCI or management of refractory no-reflow phenomena after stenting.
