Adenovirus + herpes simplex virus thymidine kinase is a gene therapy approach that uses a replication-deficient adenoviral vector to deliver the herpes simplex virus type 1 thymidine kinase (HSV-tk) gene into tumor cells. Once inside the cell, HSV-tk phosphorylates the prodrug ganciclovir (or similar nucleoside analogs), converting it into toxic metabolites that induce cell death. This system is designed as a targeted cytotoxic immunotherapy for cancer treatment—primarily high-grade gliomas and other localized malignancies—by selectively killing transduced tumor cells and potentially stimulating an anti-tumor immune response. The therapy has been evaluated in clinical trials for recurrent glioblastoma and high-grade glioma, showing improved progression-free survival and overall survival compared to conventional treatments[1][2][4]. It has also been studied preclinically in mesothelioma and lung cancer models[3].
