ADH-1 + melphalan is a combination therapy used primarily in clinical research for patients with advanced extremity melanoma. ADH-1 is a novel cyclic pentapeptide that functions as a targeted agent by disrupting N-cadherin adhesion complexes found on tumor and vascular cells. This disruption leads to increased vascular permeability and sensitizes tumor cells to chemotherapy. Melphalan is an alkylating agent (a small molecule chemotherapy drug) administered regionally using isolated limb infusion (ILI), and works by cross-linking DNA, thereby inhibiting DNA and RNA synthesis, leading to cell death. Preclinical and early clinical studies demonstrate that the combination of systemic ADH-1 with regional melphalan delivers synergistic antitumor activity, with improved complete response rates compared to historical controls of melphalan alone, although added clinical benefit has not been definitively demonstrated in larger studies. The therapy is primarily investigated for advanced, in-transit or locally advanced melanoma confined to limbs, using the route of isolated limb infusion for melphalan and intravenous administration for ADH-1[1][2][3][4].
