adrenocorticotropic hormone + prednisone is a combination therapy being evaluated in a Phase 3 clinical trial led by Mao Jianhua for the treatment of children with frequently relapsing or steroid-dependent nephrotic syndrome. Adrenocorticotropic hormone (ACTH) is a peptide hormone that acts as an agonist at melanocortin receptors. Its therapeutic effect in nephrotic syndrome is twofold: it stimulates the adrenal cortex to produce endogenous cortisol via the melanocortin 2 receptor (MC2R) and exerts direct reno-protective effects on glomerular podocytes through the melanocortin 1 receptor (MC1R). Prednisone, a synthetic glucocorticoid, complements this by providing potent systemic anti-inflammatory and immunosuppressive activity through the glucocorticoid receptor. This combination approach aims to achieve and maintain remission in pediatric patients who have failed or become dependent on standard steroid tapering protocols.
