Overview

Adriamycin + melphalan is a combination chemotherapy regimen consisting of two cytotoxic agents—doxorubicin (also known as Adriamycin) and melphalan. Doxorubicin is an anthracycline antibiotic that intercalates DNA and inhibits topoisomerase II activity, leading to DNA strand breaks and inhibition of replication in rapidly dividing cells. Melphalan is an alkylating agent that crosslinks DNA strands by forming covalent bonds with guanine bases in DNA; this disrupts both DNA and RNA synthesis and leads to cell death. The combination has been studied for synergistic effects in preclinical models such as L1210 murine leukemia and has demonstrated superior efficacy compared to either drug alone at optimal dosages[4]. Both drugs are used individually or in various combinations for the treatment of multiple myeloma and other malignancies[1][2][3][7]. This specific combination may be considered for hematologic cancers where enhanced cytotoxicity is desired.
