Overview

This is a combination chemotherapy regimen consisting of three cytotoxic agents: adriamycin (also known as doxorubicin), methotrexate, and vinblastine. Each drug has a distinct mechanism of action: - Adriamycin (doxorubicin) is an anthracycline antibiotic that intercalates DNA, inhibits topoisomerase II, and generates free radicals leading to DNA damage. - Methotrexate is an antimetabolite that inhibits dihydrofolate reductase, blocking DNA synthesis and cell replication. - Vinblastine is a vinca alkaloid that binds tubulin and inhibits microtubule formation, arresting cells in metaphase. This combination has been used in oncology for the treatment of various cancers. While the most common regimen including these drugs also contains cisplatin (the MVAC regimen), this specific three-drug combination may be considered for patients who cannot tolerate cisplatin or as part of alternative protocols. The primary indications are typically advanced or metastatic urothelial carcinoma and other solid tumors where multi-agent chemotherapy regimens are indicated[2][1].
