ADSVF + microfat is an autologous cell-based therapy combining adipose-derived stromal vascular fraction (ADSVF) with purified microfat, primarily developed for the treatment of refractory perianal fistulas in Crohn's disease. ADSVF is a heterogeneous cellular mixture obtained through the enzymatic digestion of lipoaspirated adipose tissue, containing mesenchymal stem cells (MSCs), endothelial progenitor cells, and various immune cells. Microfat is adipose tissue purified via filtration to remove excess fluids and lipids, serving as a volumizing scaffold. The therapy exerts its therapeutic effect through a multi-modal mechanism: the ADSVF component provides immunomodulatory, anti-inflammatory, and angiogenic properties that promote tissue repair, while the microfat component provides trophic support and physical bulk to facilitate the closure of fistula tracts. The combination is typically administered via local injection into the fistula wall and surrounding tissues.
