Afamelanotide + NB-UVB is a **combination therapy** for vitiligo consisting of subcutaneous afamelanotide implant (a synthetic analog of alpha-melanocyte-stimulating hormone) and narrowband ultraviolet B (NB-UVB) phototherapy. Afamelanotide acts as a **melanocortin-1 receptor agonist**, stimulating melanocytes to produce eumelanin and promoting melanocyte differentiation and proliferation, while NB-UVB phototherapy suppresses local autoimmune inflammation and stimulates melanocyte regrowth. Randomized controlled trials have shown the combination yields faster and more significant repigmentation, particularly in patients with darker skin types (Fitzpatrick IV-VI), than NB-UVB alone, with acceptable safety. Common adverse effects are erythema, diffuse hyperpigmentation, nausea, and local implant reactions.
