This drug is a **combination of aficamten (CK-3773274)**, a next-generation cardiac myosin inhibitor, and **disopyramide**, a classic Class Ia antiarrhythmic agent. - **Aficamten (CK-3773274)** is a small molecule designed as a selective, allosteric cardiac myosin inhibitor. It reduces cardiac muscle contractility by inhibiting the mechanochemical enzyme cardiac myosin, thereby directly lowering the force of cardiac muscle contraction. It was optimized for once-daily dosing, moderate half-life, and minimal cytochrome P450 induction or inhibition. Developed for hypertrophic cardiomyopathy (HCM) and related conditions to counteract cardiac hypercontractility and reduce left ventricular outflow tract gradients[4]. - **Disopyramide** is a small molecule antiarrhythmic drug of the Class Ia group. It stabilizes cardiac membrane potentials by blocking fast sodium channels, prolonging the action potential, and has anticholinergic and local anesthetic properties. It treats ventricular arrhythmias and is also used in HCM to reduce gradient through negative inotropic effects[2]. The combination is under investigation for synergistic reduction of cardiac contractility and arrhythmia burden in patients with hypertrophic cardiomyopathy or similar conditions.
