Overview

This is a combination therapy consisting of three anti-vascular endothelial growth factor (anti-VEGF) agents—aflibercept, bevacizumab, and ranibizumab—along with the corticosteroid triamcinolone acetonide. - **Aflibercept** acts as a decoy receptor for VEGF-A, VEGF-B, and placental growth factor (PlGF), preventing their interaction with native receptors on endothelial cells and thereby inhibiting angiogenesis and vascular permeability. - **Bevacizumab** is a monoclonal antibody that binds to VEGF-A, blocking its activity and reducing abnormal blood vessel growth and leakage. - **Ranibizumab** is an antibody fragment targeting VEGF-A with high affinity to inhibit neovascularization and vascular leakage in the retina. - **Triamcinolone acetonide** is a synthetic corticosteroid that reduces inflammation by inhibiting multiple inflammatory cytokines (including P-selectin and ICAM-1), stabilizing the blood-retinal barrier, lowering leukocyte–endothelium interactions, and modestly reducing intraocular VEGF levels. The rationale for combining these agents lies in their complementary mechanisms of action; anti-VEGF drugs target pathological angiogenesis while corticosteroids address inflammatory pathways contributing to retinal edema. This combination may be considered in cases of persistent or refractory diabetic macular edema (DME) or other retinal vascular diseases where monotherapy has proven insufficient[1][5]. However, this specific four-drug combination does not appear to be standard clinical practice or widely studied as a single regimen; most studies focus on dual combinations such as aflibercept plus triamcinolone[1].
