Overview

Aflibercept + ramucirumab is a combination of two anti-angiogenic agents used in the treatment of metastatic colorectal cancer (mCRC), particularly as second- or third-line therapy in combination with chemotherapy regimens such as FOLFIRI (folinic acid, fluorouracil, and irinotecan)[1][2][3]. - **Aflibercept** is a recombinant fusion protein that acts as a decoy receptor for vascular endothelial growth factor A (VEGF-A), VEGF-B, and placental growth factor (PlGF). By binding these ligands with high affinity, it prevents their interaction with endogenous VEGF receptors on endothelial cells and inhibits angiogenesis[6]. - **Ramucirumab** is a fully human monoclonal antibody targeting vascular endothelial growth factor receptor 2 (VEGFR2). It blocks the binding of VEGF-A, VEGF-C, and VEGF-D to this receptor on endothelial cells, thereby inhibiting downstream signaling required for new blood vessel formation[8][9]. Both drugs are approved for use in mCRC when combined with FOLFIRI after progression on first-line therapies. Their mechanisms complement each other by broadly inhibiting the VEGF pathway at different points.
