Overview

AGEN1571 + balstilimab is an investigational combination immunotherapy comprising two distinct monoclonal antibodies developed by Agenus. - AGEN1571 is a novel anti-ILT2 (immunoglobulin-like transcript 2) monoclonal antibody designed to modulate tumor-associated macrophages, T, NK, and NKT cells to overcome resistance to PD-1–directed therapies. It acts by polarizing macrophages to a pro-inflammatory phenotype and enhancing the activation of cytotoxic (CD8+) T cells, NK, and NKT cells within the tumor microenvironment, which may enhance innate and adaptive immune responses against tumors[5]. - Balstilimab (AGEN2034) is a fully human monoclonal IgG4 antibody against PD-1 (programmed cell death protein 1) that blocks interactions with PD-L1 and PD-L2, reversing tumor-mediated immune suppression and restoring T-cell activity against cancer cells[2][4]. The combination is under evaluation in advanced solid tumors, including microsatellite stable colorectal cancer, where it has demonstrated tolerability and promising activity in early-phase clinical trials[3][5][10]. The pairing of these agents is intended to simultaneously block key immunosuppressive pathways and enhance antitumor immunity in settings resistant to standard checkpoint inhibitors.
