**AK117 + irinotecan** is a combination therapy comprising AK117, a humanized monoclonal antibody targeting CD47, and irinotecan, a small molecule inhibitor of DNA topoisomerase I. AK117 works by blocking CD47, a "don't eat me" signal on tumor cells, thereby enhancing macrophage-mediated phagocytosis of cancer cells (immune-mediated clearance). Irinotecan is a chemotherapeutic agent that inhibits DNA topoisomerase I, resulting in DNA damage during replication and cell cycle arrest, leading to cancer cell death. This combination is under clinical investigation, typically for advanced solid tumors, including gastrointestinal cancers such as gastric, gastroesophageal junction (GEJ), esophageal, and colorectal cancers.
