Overview

Albumin + furosemide is a combination therapy involving the co-administration of human serum albumin and the loop diuretic furosemide. This regimen is primarily used in patients with edema who are resistant to diuretics alone, particularly those with hypoalbuminemia (low serum albumin levels), such as in nephrotic syndrome, liver cirrhosis, or chronic kidney disease. Albumin increases plasma oncotic pressure and expands intravascular volume, while furosemide inhibits sodium and chloride reabsorption in the thick ascending limb of the loop of Henle, promoting diuresis. The combination enhances delivery of furosemide to its site of action by increasing its binding to albumin and facilitating secretion into renal tubules. Clinical studies suggest that this combination can increase urine output and natriuresis more effectively than either agent alone in select populations[1][2][5][7]. However, response varies among patients.
