Albumin + lactated ringer's solution + normal saline is a combination fluid resuscitation strategy used in the management of sepsis and septic shock. This regimen involves the administration of human serum albumin, a colloid protein, alongside isotonic crystalloid solutions such as lactated Ringer's and 0.9% normal saline. Albumin acts to increase plasma oncotic pressure, thereby maintaining intravascular volume and reducing edema, while also providing antioxidant and anti-inflammatory benefits that protect the vascular endothelium and glycocalyx. The crystalloid components provide immediate volume expansion and electrolyte balance. Research, including a retrospective analysis of the MIMIC-IV database published in 2021, indicates that combining albumin with crystalloids within the first 24 hours of treatment is associated with significantly improved 28-day survival in septic patients compared to the use of crystalloids alone.
