Aldesleukin is a recombinant form of interleukin-2 (IL-2), a biologic cytokine that plays a critical role in the signaling and regulation of white blood cells. In the context of steroid-refractory chronic graft-versus-host disease (cGVHD), aldesleukin is being investigated by the Dana-Farber Cancer Institute as part of a novel combination therapy. This regimen involves the administration of low-dose subcutaneous aldesleukin (typically 1 × 10^6 IU/m²) to preferentially expand the population of regulatory T cells (Tregs), which possess potent immunosuppressive properties. This is combined with an infusion of donor-derived, Treg-enriched cells to further enhance the restoration of immune homeostasis and counteract the donor immune system's attack on host tissues. The approach aims to reverse the severe symptoms of cGVHD by leveraging the immunomodulatory effects of IL-2 at low doses, which paradoxically suppresses rather than stimulates the overall immune response.
