Overview

Aldoxorubicin + doxorubicin is an investigational combination of two anthracycline chemotherapy agents used together for the treatment of solid tumors, notably in clinical research settings on soft tissue sarcomas and other advanced malignancies. Aldoxorubicin is a prodrug of doxorubicin, in which doxorubicin is attached to a novel acid-sensitive linker (EMCH; N-ε-maleimidocaproic acid hydrazide), enabling rapid binding to circulating albumin. This design allows aldoxorubicin to exploit albumin uptake by tumors for preferential delivery and release of doxorubicin in the acidic tumor environment, potentially improving tumor targeting, limiting systemic toxicity (most notably cardiotoxicity), and allowing higher dosing than conventional doxorubicin[1][5][7]. Doxorubicin itself is a classic anthracycline that intercalates DNA and inhibits topoisomerase II, leading to impaired DNA replication and cell death. When combined, the dosing in clinical trials for this combination has consisted of intravenous aldoxorubicin (up to 320 mg/m2) and doxorubicin (up to 35 mg/m2) given every 21 days, with the combination being feasible and associated primarily with hematologic toxicities[3].
