Overview

The combination of alectinib, pertuzumab, and trastuzumab is a targeted therapy approach used for patients with overlapping anaplastic lymphoma kinase (ALK)-rearranged non-small cell lung cancer (NSCLC) and HER2-mutant breast cancer. This combination therapy represents a personalized therapeutic approach for managing patients with multiple malignancies. A case report documented a 71-year-old woman with both ALK-rearranged lung adenocarcinoma and HER2-mutant breast cancer who achieved significant improvement with the concurrent use of these three molecularly targeted agents[1][2][6]. The treatment protocol initially involved discontinuing alectinib and starting trastuzumab, pertuzumab, and docetaxel for breast cancer. However, when lung cancer progression occurred while the patient was on trastuzumab and pertuzumab, alectinib was reintroduced alongside the other two drugs. After six months of this triple combination therapy, the patient showed sustained reduction in both lung cancer, breast cancer, and brain metastases with no adverse events[2][6]. This combination therapy represents an innovative approach for patients with concurrent ALK-rearranged NSCLC and HER2-mutant breast cancer. The case report suggests that this concurrent administration of multiple molecularly targeted agents is a feasible and safe strategy without compromising efficacy[1][2][6]. While trastuzumab and pertuzumab are commonly used together for HER2-positive breast cancer, the addition of alectinib for concurrent treatment of ALK-rearranged NSCLC represents a novel therapeutic strategy that addresses the challenge of treating patients with multiple cancers simultaneously. The success of this combination therapy underscores the importance of personalized medicine in oncology, particularly for patients with multiple malignancies that require different treatment approaches.
