This therapeutic regimen, developed by Dr. Richard Burt, is an immunoablative strategy followed by autologous hematopoietic stem cell transplantation (HSCT) for the treatment of refractory antiphospholipid syndrome (APS). The protocol involves a conditioning phase using high-dose cyclophosphamide, an alkylating agent, and alemtuzumab (CAMPATH-1H), a monoclonal antibody targeting the CD52 antigen. This combination is designed to deplete the patient's autoreactive immune cells, particularly the B and T lymphocytes responsible for producing pathogenic antiphospholipid antibodies. Following immunoablation, previously harvested autologous hematopoietic stem cells are re-infused to reconstitute a healthy, non-autoreactive immune system. The goal of this 'immune reset' is to achieve long-term clinical remission and eliminate the risk of recurrent thrombosis and pregnancy complications associated with APS.
