Overview

Alemtuzumab + fludarabine + cyclophosphamide is a combination conditioning regimen developed by the Masonic Cancer Center at the University of Minnesota for use in allogeneic hematopoietic stem cell transplantation (HSCT). This regimen is specifically designed for patients with high-risk non-malignant hematologic disorders, including sickle cell disease, thalassemia, and various bone marrow failure syndromes like Diamond-Blackfan anemia and Shwachman-Diamond syndrome. Alemtuzumab is a humanized monoclonal antibody that targets the CD52 antigen on lymphocytes, providing potent in vivo T-cell depletion to prevent graft-versus-host disease (GVHD) and graft rejection. Fludarabine, a purine analog, and cyclophosphamide, an alkylating agent, provide the necessary myelosuppression and additional immunosuppression to allow for donor cell engraftment. In the clinical trial context (NCT01015131), this combination (often referred to as Regimen A2) is typically administered alongside low-dose total body irradiation (TBI) to achieve stable mixed chimerism with reduced toxicity compared to traditional myeloablative conditioning.
