This reduced-intensity conditioning (RIC) regimen is a multi-drug protocol developed by Dr. Randy Windreich for pediatric and young adult patients with high-risk acute myeloid leukemia (AML) or myelodysplastic syndrome (MDS) undergoing allogeneic hematopoietic stem cell transplantation (HSCT). The regimen is specifically designed to minimize transplant-related toxicity, thereby expanding eligibility for HSCT to patients with significant pre-transplant comorbidities. It consists of alemtuzumab (Campath), hydroxyurea (Droxia), fludarabine (Fludara), melphalan (Alkeran), and thiotepa. Alemtuzumab provides potent lymphodepletion to prevent graft-versus-host disease and facilitate engraftment, while the combination of antimetabolites and alkylating agents provides the necessary cytoreduction and immunosuppression with a lower toxicity profile than traditional myeloablative regimens.
