Alendronate + alfacalcidol is a fixed-dose combination therapy used primarily for the treatment of osteoporosis, especially in postmenopausal women and men at increased risk of fractures. Alendronate is a bisphosphonate that inhibits bone resorption by binding to bone hydroxyapatite and inducing osteoclast apoptosis, thereby reducing bone turnover and increasing bone mineral density. Alfacalcidol is a vitamin D analogue that acts as a regulator of calcium and phosphate metabolism; it is rapidly converted in the liver to calcitriol (1,25-dihydroxyvitamin D), which increases intestinal absorption of calcium and phosphate, supports proper bone mineralization, and reduces fall risk by improving muscle function. Clinical studies have shown that this combination provides superior efficacy in increasing lumbar spine and hip bone mineral density, reducing fracture incidence, lowering fall rates, and alleviating back pain compared to either agent alone or combinations with plain vitamin D[1][4][5].
