Alendronate + denosumab is a combination therapy consisting of two antiresorptive agents used in the management of osteoporosis and other bone loss disorders. Alendronate is a nitrogen-containing bisphosphonate that inhibits farnesyl-pyrophosphate synthase, leading to reduced osteoclast-mediated bone resorption. Denosumab is a fully human monoclonal antibody that binds to and inhibits RANKL (receptor activator of nuclear factor kappa-B ligand), thereby preventing osteoclast formation, function, and survival. The combination may be considered in certain clinical scenarios where monotherapy does not achieve adequate response or for sequential/adjunctive use in high-risk patients[1][5][6][8].
