This combination therapy is a multi-drug regimen being evaluated for the treatment of refractory chronic osteomyelitis, specifically Cierny-Mader Type I and III. The regimen consists of alendronate, a nitrogen-containing bisphosphonate, administered alongside standard antibiotic therapy. Alendronate acts by inhibiting osteoclast-mediated bone resorption through the inhibition of farnesyl pyrophosphate synthase, which may help preserve bone architecture and potentially enhance antibiotic penetration or efficacy within the bone microenvironment. The antibiotic component includes vancomycin, a glycopeptide that inhibits bacterial cell wall synthesis by binding to the D-Ala-D-Ala terminus of nascent peptidoglycan, followed by sequential oral linezolid, an oxazolidinone that inhibits protein synthesis by binding to the 50S ribosomal subunit. This approach is currently being studied in a Phase 3 clinical trial led by the Second Affiliated Hospital, School of Medicine, Zhejiang University.
