Overview

Alirocumab + lipid-modifying therapy is a combination regimen in which **alirocumab**, a fully human monoclonal antibody targeting proprotein convertase subtilisin/kexin type 9 (**PCSK9**), is used together with another lipid-lowering medication (typically a statin, but can also include agents like ezetimibe or fenofibrate). Alirocumab binds with high affinity and specificity to PCSK9, preventing it from degrading low-density lipoprotein receptors (**LDLR**) on hepatocytes. This results in increased LDLR availability on the liver cell surface, promoting greater clearance of low-density lipoprotein cholesterol (LDL-C) from the plasma. The addition of alirocumab to background lipid-modifying therapy has been shown to result in profound and sustained reductions in LDL-C, total cholesterol, apolipoprotein B, and lipoprotein(a), with additional benefits seen across a range of lipid fractions. This approach is particularly indicated for patients with hypercholesterolemia (including familial forms) and those at high or very high cardiovascular risk who do not reach lipid targets on standard therapy alone[1][2][3][5].
