The regimen utilizes three distinct mechanisms to inhibit the renin-angiotensin-aldosterone system (RAAS): telmisartan acts as an angiotensin II receptor blocker (ARB), aliskiren serves as a direct renin inhibitor, and eplerenone functions as a mineralocorticoid receptor antagonist (MRA). By targeting multiple points in the RAAS cascade, the combination aims to achieve more complete suppression of the system than monotherapy or dual blockade, potentially leading to greater reductions in proteinuria and improved renal outcomes. This specific triple-blockade approach was evaluated in a Phase 4 clinical trial sponsored by the Medical University of Gdansk.
