Overview

Aliskiren + ramipril is a fixed-dose combination of two antihypertensive agents used for the treatment of high blood pressure (hypertension). Aliskiren is a direct renin inhibitor that blocks the activity of renin, an enzyme involved in the first step of the renin-angiotensin-aldosterone system (RAAS), thereby reducing plasma renin activity and decreasing levels of angiotensin I and II. Ramipril is an angiotensin-converting enzyme (ACE) inhibitor that prevents the conversion of angiotensin I to angiotensin II, leading to vasodilation and reduced blood pressure. The combination provides greater reductions in systolic and diastolic blood pressure compared to either agent alone by targeting different points within the RAAS pathway. However, dual blockade increases risks such as hyperkalemia, hypotension, and renal impairment; thus, its use requires careful monitoring and is generally not recommended in patients with diabetes or kidney impairment[1][4][7][9].
