Overview
Alisporivir + peginterferon alfa-2a is an investigational **combination regimen** for the treatment of chronic hepatitis C virus (HCV) infection. **Alisporivir** is a small molecule, orally administered cyclophilin inhibitor, which impairs hepatitis C viral replication by targeting the host protein cyclophilin A, thus providing pangenotypic antiviral activity and a high barrier to resistance. **Peginterferon alfa-2a** is a pegylated recombinant form of human interferon alfa-2a that acts primarily by stimulating the innate antiviral immune response through activation of the type 1 interferon receptor and the JAK-STAT pathway, increasing expression of antiviral genes. This combination, often given with ribavirin, has been studied in phase 2 and phase 3 trials, showing improved sustained virological response rates compared to peginterferon alfa-2a plus ribavirin alone, especially in HCV genotype 1 patients. Development was primarily led by Novartis, but alisporivir's clinical advancement was placed on hold due to safety concerns.
